We schedule patients from 8:00 a.m. until 5:00, 6:00 p.m. on Mondays, so that missed school or work is kept within reasonable limits. Accordingly, each patient is offered a fair portion of early or late appointments. Your time and convenience are important to us.
Please arrive on time for your appointment. If you are more than 15 minutes late, you may be asked to reschedule. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.
We realize unexpected circumstances arise and you cannot keep your appointment. If this occurs, we ask that you give us 48 hours notice to cancel or reschedule your appointment.
We accept cash, checks, Visa, Mastercard, Discover and American Express.
We never want costs to stand in the way of quality treatment, which is why we are dedicated to providing our patients with care that is both effective and affordable. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We accept payment by cash, checks, and all major credit cards. We offer flexible payment plans with 0% In-house financing as well as care credit. We also offer payment in full discounts as well as multiple family member discounts.

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.
When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within a reasonable timeframe. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
